A copy of a photographic ID must be submitted with request. The ID must be a valid, government issued document. If not available, you may submit photocopies of two other forms of identification.

City/Town issued Certified Certificates may also be available. Contact the city or town. Make personal check or money order payable to the town or city where the birth occurred.
